To submit your gemstone to GIA, print the documents below and include in the package with your gemstone. If you don’t have access to a printer, you can email a copy to yourself and print later. Please do not email this form to GIA as we can only process your order once we receive the gemstone and the order form together.

Please do not write company name, initials, logos, phone, fax, email, website or any other information that identifies you as the client or refers to grading information on your parcel papers.
