On Campus Application

Thank you for applying to GIA’s on campus programs.  You can open the PDF forms below and type your information directly on to them, then just print them out, sign them and return them to GIA. All you need is the free Adobe Acrobat Reader (Version 7) to get started!

1.  Before completing your application, please print out and read these instructions.They contain important information about degree and diploma requirements, and refund and cancellation policies. You will also find information about application fees, tuitions and class schedules, and any additional documents you may need to supply to complete your application.

2. If you’ve never applied to GIA before, please complete an Application for Admission, then proceed to step 3.

3. For on campus gemology, design, or jewelry manufacturing courses or programs, please complete the School of Gemology/ School of Jewelry Manufacturing Arts Registration Form. For on campus business courses or programs, including the Bachelor’s of Business Administration degree program, please complete the School of Business Registration Form.

Important note: Please make your entries carefully and double check your totals. It is possible to select invalid course and date combinations.

Once you’ve completed and signed the application and registration forms, please send them to GIA along with any applicable payments. You can either:
